Terms & Conditions*
- I am adopting the pet described below through Rescue Garage. The pet is being adopted as a companion and will live as a family member with me inside my home. I will not allow the pet to be used for purposes of vivisection or experimentation of any kind.
- I will provide proper and sufficient, food, water, shelter and kind treatment for the pet at all times. I further agree the dog will not be chained at any time as a means of restraint or be a primarily outside dog.
- I will keep a collar and identification tag on the dog at all times and will license the dog with the county/city. And will also keep the microchip information current for this dog.
- I agree to keep this dog on regular heart worm prevention.
- I agree that a representative of Rescue Garage may make periodic inspections of the pet, my residence and the pet’s living conditions on reasonable notice. I further agree to surrender the pet to Rescue Garage's representative if for any reason he/she considers the conditions provided by me for the pet to be unsatisfactory.
- I agree to immediately notify Rescue Garage if the pet is lost or stolen. I further agree, in the event the animal is lost, that I will make every reasonable effort to attempt to locate and claim the animal, and to immediately retrieve the pet from any public pound or animal shelter when notified the animal is being held at any such location and to pay any costs associated with its recovery.
- Rescue Garage has first right of refusal to accept the animal back into rescue. I agree to allow Rescue Garage reclaim the animal, if upon investigation, the animal is not being properly cared for or if I have not complied with the provisions of the agreement. I agree to pay all costs necessary to enforce this application including reasonable attorney fees and all costs of litigation. I understand that animals can be unpredictable and Rescue Garage cannot anticipate or ensure unwanted behavior of animals adopted from Rescue Garage. I hereby accept the animal as is, assume all risks and responsibilities associated with ownership of the animal, including bites, and I hereby fully and completely release, indemnify, and hold harmless Rescue Garage, its directors, employees, and volunteers from any claim, cause or action, or liability of any sort of nature, whether known or unknown, directly arising out of or in connection with the adoption, care or ownership, maintenance, temperament, conduct or condition of the animal.Your initials acknowledge the aforementioned
- I agree that in the event I can not keep the animal I understand I MUST return the animal to Rescue Garage.
